It’s time to start the conversation! Time to decide what healthcare decisions we would want if we are unable to make them ourselves.

2. What would you want if you were in a situation like that?
3. Would you want life sustaining measures, such as CPR, tube feeding, ventilator support or antibiotics if you had:
   • An incurable illness?
   • Permanent severe brain damage with no expectation of recovery?
   • An irreversible chronic illness, i.e. Alzheimer's Disease?
4. Where would you want to spend your last days?
5. Who would you want to be with you?
6. Who would you want to make health care decisions for you if you were unable to do so? Would you also choose an alternate? Who?
